good question...i saw this past interview of Chow Yun Fat, in which he pointed out how he purposely added little facial/body gestures to his performance, such as tilting the corner of his lips, flipping a coin between fingers...etc.
the unique yelling sound during fights and the nose rubbing gesture of Bruce Lee, happen to be shown in most of his later films which i really doubt if they were written in any of the movie scripts.
therefore, i
would assume the creepy laughter and the "lam" 俐 gesture were ledger's personal interpretation of the character...but movie making is a group activity, i am sure there were group discussions during the filming process among the actor himself, scriptwriter, director and producer to give the "go ahead", especially in Hollywood.
but chances are "the tongue licking" act was actually part of the script...maybe either you or I should give producer "Chris Nolan" a call to clarify on that. LOL....
